Position Summary

Personal Care Assistance/Caregiver

The Personal Care Assistant/Caregiver provides compassionate, dependable, non-medical support to clients in their homes. This position promotes independence, comfort, safety, dignity, and emotional well-being.

Role Details

Key Responsibilities

Care is provided according to the authorized service plan, agency policies, and applicable requirements.

Personal & Daily Support

  • Assist with bathing, dressing, oral care, hygiene, and toileting.
  • Assist with mobility, transfers, repositioning, and ambulation.
  • Provide medication reminders within the authorized scope of service.
  • Provide respite care and appropriate safety supervision.
  • Assist with meal planning and preparation.
  • Perform light household tasks related to client care, including laundry, dishes, and trash removal.

Safety, Communication & Documentation

  • Assist with personal shopping and scheduling appointments.
  • Assist with approved calls, texts, or emails with the client’s permission.
  • Provide companionship through conversation, reading, games, walks, movies, and mealtime engagement.
  • Provide authorized transportation to appointments, errands, and community outings.
  • Follow the individualized care plan and document daily services accurately.
  • Promptly report changes in the client’s condition, behavior, or environment.
  • Perform other assigned duties within the authorized scope of the position.
Qualifications

Required Qualifications

  • At least 18 years of age.
  • High school diploma or GED.
  • Reliable transportation and verifiable automobile insurance when driving is required.
  • Ability to satisfy applicable background-screening and agency drug-screening requirements.
  • Strong communication skills, dependability, and professional conduct.
  • Ability to work independently and follow authorized care plans.

Preferred Qualifications

  • Previous experience as a PCA, caregiver, HHA, or CNA.
  • Current CPR and First Aid certification.
  • Dementia or Alzheimer’s care experience.
  • Experience with electronic visit verification, including HHAeXchange.
  • Previous one-on-one home-care experience.

Position Summary

Home Health Aide (HHA)

The Home Health Aide provides hands-on personal care and basic health-related services to clients under the supervision of a licensed nurse or agency designee. HHAs support clients in maintaining independence and safety within their homes.

Care is provided according to the authorized service plan, agency policies, and applicable requirements.

Personal & Daily Support

  • Assist with personal hygiene, bathing, grooming, and dressing.
  • Assist with mobility, transfers, positioning, and ambulation.
  • Take and record vital signs when permitted, assigned, and within the role’s authorized scope.
  • Assist with prescribed exercises and range-of-motion activities.
  • Provide medication reminders only; do not administer medication unless authorized and permitted.
  • Provide light housekeeping and meal preparation related to the client’s care.

Safety, Communication & Documentation

  • Observe and promptly report changes in the client’s condition to supervising staff.
  • Maintain complete, accurate, and timely care notes and service records.
  • Follow infection-control practices, safety procedures, emergency protocols, and HIPAA requirements.
  • Communicate professionally with clients, families, supervisors, and care team members.
  • Follow the individualized care plan and instructions provided by the supervising professional.
  • Perform other assigned duties within the authorized scope of the HHA position.
